The 5 out action starts with the 5-man centered with the ball at the top of the key and the other 4 players around the perimeter.
The two most common ways to get into 5-out action are after a Chin flare screen tight curl, and a REEL flare screen.
Here is a 5-out alignment that starts with a dribble at the wing, backcut, and DHO with the guy in the corner. You'll notice the guard cutting through gets a screen at the elbow on the other side of the floor.
